What an evaluation can include

facility

Canonical facility name, operator, address, state, city, slug, and reviewed identifiers.

coverage

Published source-record presence, factual record counts, and an explicit unknown status when no reviewed search is documented.

source_records

System, adverse flag when supported by the source, source URL, as-of date, and the provenance fields present in the supplied sample.

matching

Match confidence and review routing labels for submitted portfolios and evaluation extracts.

Designed for auditability

Every returned record carries its source and retrieval context. Search coverage is labeled unknown unless a separate record confirms that source was searched.

Evaluation guardrails

Entity-only

The evaluation reports on businesses and facilities, not natural persons.

Source-backed

The public sample includes source URLs, as-of dates, explicit coverage limitations, and a list of supplied provenance fields.

Evaluation-only

A sample does not promise production delivery, recurring refreshes, or unrestricted redistribution.
